Disclosures

Transparency about who provides your care.

We believe you should know exactly who you're working with. This page includes licensing details, credentials, scope of practice, and provider-specific disclosures for the practice team.

Washington disclosure readiness checklist

Website status: provider profile content is now present for Tonya and Rahab. Andrea's approved profile and disclosure packet are still pending.

Washington's counselor, marriage and family therapist, and social worker disclosure rules require written disclosure information before a treatment plan is implemented and at the commencement of treatment. The disclosure must be specific to the treatment service, easy to understand, and detailed enough for the client to make an informed decision.

The required Washington disclosure packet should include:

  • Practice or firm name
  • Business address and telephone number
  • Washington state license number
  • Licensee name
  • Methods, treatment modality, and therapeutic orientation
  • Education and training
  • Course of treatment, when known
  • Billing information, including per-session cost, billing practices, advance payments, and refunds
  • Client right to refuse treatment and choose a practitioner and treatment modality
  • Washington Department of Health complaint-process contact information
  • Signatures and dates from both the client and the licensee acknowledging the disclosure

Associate-specific requirement: Tonya's disclosure must also state that she is an associate practicing under the supervision of an approved supervisor. Associates may not independently provide marriage and family therapy for a fee.

Still needed before launch sign-off:

  • Washington credential or license number for each provider
  • Tonya's approved supervisor name and required supervision disclosure details
  • Final practice business address and office telephone number
  • Final billing/session costs, refund language, and advance-payment practices
  • Confirmation of each provider's accepting-new-patients status and SimplePractice routing
  • Final signed disclosure / informed-consent workflow used at intake
  • Final clinical/legal review of this website summary and the actual signed disclosure forms

Washington Department of Health complaint contact

Washington disclosure rules require the Department of Health complaint-process contact information to be available to clients. The current DOH complaint contact information reviewed for this draft is:
